From Client Users (as defined in the FindWellness Terms of Service), FindWellness may collect certain Personal Health Information that the Client chooses to provide. This information constitutes an individual-controlled electronic record of the Client’s health information by which the Client can manage, track, and participate in his or her own health care (a “Personal Health Record”). Client Users have full control over this Personal Health Record, and may access it at any time; receive a copy of it; update, modify, or delete inaccurate entries; and authorize or de-authorize others’ access to it, as detailed in Section 3.
From Provider Users (as defined in the FindWellness Terms of Service), FindWellness may collect general information such as level of certification and specialization of practice, but we do not collect Personal Health Information about Clients from Provider Users.
FindWellness uses User Personal Information to perform and provide the Services requested by the User. FindWellness uses User Personal Information to respond to the User’s requests and determine suitable products, services, or venues, according to the User’s preferences. FindWellness also uses User Personal Information to populate online profiles.
FindWellness may also use User Personal Information to communicate with Users. For instance, FindWellness may use information you provide to send you information regarding our Services, such as promotions or events.
FindWellness may use aggregated, non-personally identifying information gathered from our web server to operate, improve, and optimize our Website and Services. We may also use aggregated, anonymized information for demographic purposes.
FindWellness will not, nor will we allow any third party to, use the Google Analytics tool to track our Visitors; collect any User Personal Information other than IP address; or correlate your IP address with the identity of any User. FindWellness Users may update, alter, or delete their basic user profile information by editing their user profile or contacting firstname.lastname@example.org.
FindWellness will retain User Personal Information for as long as your account is active or as needed to provide you services. Users may update, modify, or delete their User Personal Information. If you wish to cancel your account, delete your User Personal Information, or request that we no longer use your information to provide you services, please contact email@example.com. We will retain and use your information as necessary to comply with our legal obligations, resolve disputes, and enforce our agreements, but barring legal requirements, we will delete your full profile within 30 days.
FindWellness may occasionally send Users an email to tell you about new features, solicit feedback, notify you of important changes, or just keep you up to date with what’s going on with FindWellness and our products. You may manage your receipt of these communications by clicking on the “unsubscribe” link located on the bottom of FindWellness’s emails or manage your notifications options in your User Profile. Additionally, you may send a request specifying your communications preferences to firstname.lastname@example.org.
Although most changes are likely to be minor, FindWellness may change its Privacy Statement from time to time, and in FindWellness’s sole discretion.
